Cuny-zink NATIONAL LEAGUE player waiver

Cuny-Zink Softball National League Player Release/Waiver of Liability and Indemnity Agreement

  1. I and my heirs, assigns, and family hereby release and hold harmless and forego the right to file suit in any court of law Cuny-Zink Softball and any other sponsoring organizations, related vendors, business entities, players, managers, league presidents/commissioners/directors, and any other personnel involved in this program from all liability claims or expenses for any injury or damage which may arise.
  2. I am physically able to participate in team play and practice. I am also aware of - and assume the responsibility for - the inherent risks, dangers, and (possibility of) serious or significant injuries that are associated with playing softball, including injuries from running, colliding with another player or object, and particularly those related to the use of slow pitch (BPF 1.20) and senior softball bats (BPF 1.21) and batted balls.
  3. I am aware and have read the Cuny-Zink Softball League Rules and Code of Conduct, and I knowingly and freely assume all such risks and responsibilities, both known and unknown, even if arising from the negligence of players or others in attendance at Cuny-Zink Softball games or other events in coming to, participating in, and leaving the venue of games and practices, and assume all risk for my participation. 
  4. I acknowledge that the members, officials, players, managers and league presidents/commissioners/directors of Cuny-Zink Softball are released from any and all liability claims due to injury or loss of personal property resulting from my participation in Cuny-Zink Softball. 
  5. I am aware that the league may elect to use photography, audio, and video of games and related league activity, and hereby voluntarily permit the use of my name, image, words, actions, and likeness for use in league communication and promotion. 
  6. I acknowledge this agreement through written (player application form) or digital signature (at Cunyzinksoftball.com) before practicing or playing games with Cuny-Zink Softball.
